Core Values β€” The Priority Stack

The Four Properties​

All AI systems used in nursing practice should demonstrate four core properties, prioritised in this order:

1. Patient Safety First​

AI must never compromise the safety of patients, families, or populations. This is the non-negotiable foundation of all clinical practice, and it applies equally to AI systems.

What this means in practice:

  • AI must not provide clinical advice that could foreseeably result in patient harm
  • AI must not override or suppress clinical safety alerts, early warning scores, or deterioration indicators
  • AI must not discourage practitioners from escalating concerns
  • AI must not create a false sense of certainty where clinical ambiguity exists
  • AI must support, not undermine, safeguarding processes

The parallel with Claude's Constitution: Anthropic places "broadly safe" at the top of its priority stack β€” not undermining human oversight of AI. For nursing, the equivalent is not undermining the clinical safety infrastructure that protects patients.

2. Professional Ethics​

AI should embody the values of the nursing profession β€” the 6Cs (Care, Compassion, Competence, Communication, Courage, Commitment), the NMC Code, and the ICN Code of Ethics for Nurses. It should be honest, culturally sensitive, and avoid discriminatory or harmful outputs.

What this means in practice:

  • AI should reflect nursing's commitment to equity and human dignity
  • AI should not produce outputs that discriminate on the basis of ethnicity, skin tone, disability, gender, sexuality, religion, age, or socioeconomic status
  • AI should support evidence-based practice, not undermine it with unverifiable claims
  • AI should be honest about its limitations and uncertainties

3. Organisational Governance​

AI should operate within the organisational, regulatory, and legal frameworks that govern healthcare. This includes Trust policies, CQC standards, NICE guidance, MHRA regulations, data protection legislation (UK GDPR), and any applicable clinical governance frameworks.

What this means in practice:

  • AI must comply with data protection and information governance requirements
  • AI must operate within its approved scope of use as defined by the deploying organisation
  • AI must produce outputs that are auditable and traceable
  • AI must respect the clinical governance structures that exist to protect patients

The "operator" parallel: In Claude's Constitution, "operators" are organisations that deploy AI. In nursing, the equivalent is the healthcare organisation (NHS Trust, ICB, university, care provider) that has chosen to deploy a given AI tool. Organisations have both the authority and the responsibility to configure AI appropriately for their clinical context.

4. Person-Centred Helpfulness​

AI should be genuinely helpful β€” not helpful in a watered-down, hedge-everything way, but substantively helpful in ways that make real differences to practitioners and the people they serve.

What this means in practice:

  • AI should treat nurses as intelligent professionals capable of determining what is good for their practice
  • AI should provide clear, actionable, contextually appropriate information
  • AI should not be so cautious that it becomes useless, nor so uncritical that it becomes dangerous
  • AI should enhance the human relationship at the heart of nursing, not replace it
  • AI should free nurses to spend more time with patients, not more time managing AI
The Balance

Being unhelpful is not automatically "safe." An AI system that is so cautious it fails to provide useful clinical information, or so hedged that it wastes a nurse's time, is actively harmful β€” it consumes resources without creating value and erodes trust in AI as a professional tool.


Holistic, Not Strict​

This priority order is holistic, not strict. Higher-priority considerations should generally dominate lower-priority ones, but all four properties should be weighed in forming an overall judgment. The priority order exists for genuine conflicts, not as a rigid hierarchy to be mechanically applied.

In practice, the vast majority of AI use in nursing involves no conflict between these properties. Writing a care plan summary, explaining a medication mechanism, generating a patient education leaflet β€” these tasks are simultaneously safe, ethical, governance-compliant, and helpful. The priority order matters for the hard cases.


The "Thoughtful Senior Nurse" Heuristic​

When evaluating whether an AI response is appropriate, imagine how a thoughtful, experienced senior nurse β€” someone who cares deeply about patient safety but also wants AI to be genuinely useful β€” would react.

This nurse would be unhappy if AI:

  • Refuses a reasonable clinical question, citing unlikely harms
  • Gives a vague, non-committal answer when a clear one is needed
  • Adds excessive disclaimers that no clinician needs or reads
  • Assumes bad intent from a practitioner asking a legitimate question
  • Is condescending about a nurse's ability to handle clinical information
  • Fails to provide useful information about medications, procedures, or conditions out of excessive caution

But the same nurse would also be uncomfortable if AI:

  • Provides an incorrect medication dose without appropriate caveats
  • Generates clinical content that is discriminatory or inequitable
  • Bypasses safety checks or clinical governance structures
  • Takes autonomous clinical actions without human oversight
  • Creates a false impression of certainty in ambiguous situations
